Hello! My name is Aimee Soar, I am a registered nurse and I am the founder of Frailty Nurse Consultants.
Frailty Nurse Consultants has been launched to provide support to family members with ageing parents and relatives, who can see their loved one is declining in health, struggling around the house and becoming increasingly frail and vulnerable.
We aim to provide support to relatives who are unsure of where to turn, who to trust or how and where to look for all the different services they may be entitled to (and may not have access to the internet).
We can also support busy individuals who have limited time to make lots of phone calls and spend hours researching and looking at all the different options.
Frailty Nurse Consultants offers a home based assessment of concerns and needs, covering all areas of health and social care. Examples of which are-
* Advice on supporting the elderly
* Social Support- Discussing options around care, housing, long term care, carers, day centres, respite.
*Ageing
* Dementia Support
* Signposting – navigating the complexities of health and social care
* General Decline in Health
* Safety – adaptation/home modifications, equipment, medications/falls.
* Medications Advice
* Carer stress, and guilt
* Continence Advice
* Benefits entitlements
* End of Life Planning
We then pull together a plan of action, with the aim of continuing to keep your relative at home safe and well cared for, discussing all options and services available. We can then, if requested follow up all recommendations on your behalf.
